Membership Setup Form
This form is required for merchants who will be using the Membership / Subscription billing application in the Online Commerce Suite.
ACH Underwriting Application
This form is required for merchants to request non-standard processing of ACH transactions and outlines the documentation required prior to beginning the underwriting review.
ACH Underwriting Proposal
This form is for use by the Reseller to outline their recommendations for the terms they would like for the merchant. The form also provides a space for the reviewer to report the outcome of the underwriting.
ACH Underwriting Fee Authorization
This form provides authorization to debit the appropriate ACH Underwriting Application Fee. A signed form is required prior to beginning the underwriting review.
